 About Your Visit
Insurance Information
We participate in most managed care plans including Oxford, United Health care, Cigna, Empire Blue Cross, and others. We also participate in Medicare. Each managed care plan has its own co-payment amount. Before your visit, please call your insurance plan to find out if outpatient mental health benefits are available to you.
Appointments
Our practice is open Monday though Thursday, from 5:00 AM to 4:30 PM. Later appointments are made on an individualized basis. If we participate in your insurance plan, we will accept payment from them for your visit. All co-payments and deductibles must, by law, be collected on the day of your visit. There can be no billing for co-payments or deductibles. Our office requires 48 hours notice for cancellation on all appointments. If not, there will be a cancellation charge. This fee is not reimbursable by insurance.
Fees & Payments
We accept cash, personal checks, and credit cards. Unfortunately, we are unable to accept checks on your first visit. Amex, Visa and MasterCard are accepted for amounts $50 and over. There are several ATM machines within a 2 block radius of the office.
Prescriptions & Renewals
If possible, Dr. Laakso will provide professional samples of any medication recommended. Be sure to take medication exactly as prescribed, report any adverse reactions and obtain any follow-up blood tests as recommended. Prescription renewal requests must be made during regular office hours, as it requires a review of your chart and recent blood tests, if needed.
Forms, Letters and Notes
Forms for re-imbursement at the time of your visit have no charge. There will be a charge for all forms not directly related to re-imbursement for the visit, such as for disability, etc. Legal opinions, reports and letters or notes to various agencies, departments and non-medical professionals will be charged depending on the time required and its complexity.
